School Health Services
The school nurse, school physician and other school personnel work together to plan and implement a comprehensive school health program. If you have any information concerning your child's health that would be beneficial for the school health team to know or have any questions concerning the health of your child, please call the school nurse to schedule an appointment at 631.727.5900, ext. 316.
Immunization records must be presented upon registration to school. The New York State immunization requirements for school attendance are available in the registration material and/or from the school nurse. No religious exemptions are accepted for required immunizations.
Physical examinations are required upon entrance/registration to school, as well as seventh and tenth grades and at any other time deemed necessary by school authorities. Physical forms for completion by your private health care provider are available from the school office. You can also download the physical form. During the school year, the school physician will be conducting school physicals for students who need this service to comply with state requirements. This examination is only a screening procedure and not a diagnostic exam. If a condition is found that needs further evaluation, the parent will be notified, and a health form, to be completed by your private health care provider, will be required.
Health screenings are done annually and include height, weight and vision for grades seven through twelve. Hearing screenings are done in seventh, eighth and tenth grades and for all students participating in interscholastic sports. Scoliosis screenings are done for students in seventh through tenth grades.
